ALL TEAMS ARE REQUIRED TO DO CHECK IN ON FRIDAY NIGHT, NO EXCEPTIONS.

All of these forms can be found in Forms & FAQ.  All are required.  There is absolutly no exception to the fact our medical release form must be completed and signed by every parent/ guardian.  All forms turned in must be the originals, we strongly suggest you make copies to keep prior to arriving.

Permission to Travel (our Permission to host is on our site)

Copy of your team Roster from Demosphere (ONLY)

Please make sure to follow the instructions on the cover sheet/ form.  It details the order in which all paperwork should be once you are ready to check in.  We ask that you do not come to the check in desk until you are ready with the above in order to make this process to as quickly as possible for all teams.
